Designed with parents of young children in mind, our β€œMy Child’s Speech Skills” is an informal assessment tool used as part of a speech assessment. It is an invaluable tool for gathering information about a child’s speech development from parents before their first session or from observations in play-based assessment. This facilitates good communication between parents and SLPs, which can set you up for building a good therapeutic relationship.

Are you looking for a simple, one-page sheet that allows you to record a child’s sound inventory, as well as HOW they say their words?

This summary can be useful to send to parents before your initial appointment so that you have some information on their speech skills, or it might be useful for a more play-based session with the SLP completing the form.

IT MIGHT BE USEFUL:
βœ… — for children between 2-3 years of age
βœ… — when a child is reluctant to speak in a session and you’d like more information on their speech skills
βœ… — to send prior to starting speech therapy
